PDF Bookmarks: Can you create a pdf bookmark that includes several pages?

The standard pdf bookmark format is one bookmark per page at the top level (with any in-page bookmarks that were added grouped by page):

1: Page 1 Title
Shape 1
Shape 2
2: Page 2 Title
Shape 1
Shape 2

Is there are way to set up a pdf bookmark that includes several pages?  For example:

Section 1
1: Page 1 Title
Shape 1
Shape 2
2: Page 2 Title
Section 2
3: Page 3 Title
4: Page 4 Title

 

I can always do it in Acrobat afterwards, but it would be nice to have it export that way...
