Change Units in Property Bar to "kilometers". Double click a ruler (opens the Rulers section in Options) and then click "Edit Scale...". Set "Page distance" to 1 meter (for example) and "World distance" to 1 kilometers.The maximum page size will now be 45.72 x 45.72 kilometers.
